Our office schedules appointments for pre-employment physical exams, immigration medical exams, worker's compensation injuries.  

For pre-employment physical and immigration exams, you may contact your insurance carrier to inquire as to whether or not this physical exam.  Please contact them prior to your scheduled appoiontment.  In most cases, if you have had a physical exam within the past 12 months, insurance carriers will not pay for an additional physical.

DOT (Department of Transportation Exams) 

$100 Includes physical for $75 and urinalysis for $25.  The urinalysis is NOt a drug screen test. 
We have all necessary DOT forms.

Upon employer request (additonal fees):
Drug screen $50
Alcohol testing  $25

IMMIGRATION EXAMS

FEES:

EXAM AND BLOOD WORK/TB SKIN TEST:
$145
(includes exam, HIV and syphyllis blood testing, TB skin test.  Blood work is required on children ages 14 and older.  

IMMUNIZATIONS: (additonal fees if to be administered)
$  80  MMR 
 $40  TETANUS
 $100  VARIVAX
 
CHILDREN FEE: (under age 14)
$100  with prood for immunization record; includes exma and TB skin test

ADDITIONAL FEE(S):
$50  to complete necessary paperwork including bloodwork, immunizations

(?) Additional $75 (HIV confirmation)
This additional fee will be required if the first HIV test is positive.  The lab automatically performs this confirmatory blood test to rule out the HIV virus.

CHEST XRAY  If the TB skin test is positive, the patient will be sent to another facility for a chest xray which will be at their expense.

RESULTS:
Within 48 - 72 hours when TB skin test reading is performed; same day to complete paperwork only.
